Orthopedic Shoes

• Core plan: 70% coverage

Based on a rolling 2-year period, you and your dependants each have a maximum of $1,500 (Enhanced) or $750 (Core) towards the purchase of orthopedic shoes, including repairs.

Requirement(s): 

• Orthopedic shoes must be custom made, individually fitted, and are required due to anatomical deformity. 
• Orthopedic shoes must be prescribed by a physician, podiatrist, chiropractor, or nurse practitioner.
• Orthopedic shoes must be purchased from a podiatrist, pedorthist, or a registered orthotist.
• The following must be provided at the time of claim submission: 
  • A written order/prescription.
  • A completed fabrication form signed by a podiatrist, pedorthist, or registered orthotist.
  • A receipt of purchase.

Exclusion(s):

• Off-the-shelf products (e.g. Birkenstock, Blundstone, etc.)
